Transitional Care Program

The Transitional Care Program at Franklin Hospital offers a place for you to rehabilitate closer to home in a caring skilled nursing environment.

“Swing Bed” is a Medicare program designed to provide additional skilled care to those needing extra time to heal before returning home. You do not need to be hospitalized at Franklin Hospital to enter our Transitional Care Program. You can request to be sent here from any medical facility following your hospitalization or surgery.

Requirements

- A physician’s order for “Swing Bed evaluation” with qualifying diagnosis

- Three (3) consecutive midnight stays as an inpatient in an acute care facility

- Benefits through Medicare based on diagnosis and need of skilled nursing or rehabilitation services

Who can benefit

-Patients recovering from major surgery, joint replacement, abdominal and vascular surgical procedures.

-Patients recovering from an accident or stroke.

-Patients with wounds that need special care.
